To download your records:

1. Log into your AthenaHealth patient portal.
   https://23812.portal.athenahealth.com/
2. Select My Health on the left menu.
3. Click the Forms and Documents tab on the top menu.
4. Right-click on the signed document titled:
   Historical Medical Record Cover Sheet – One Patient
5. Select Open Link in New Tab

It’ll feel like is stuck, but, please be patient, it is a large file to process.

Once done, you’ll be prompted to save the pdf file to your computer.
